Out-of-Network Claim Form
A completed and signed form is required with submission of all out-of-network claims if the provider is not submitting a claim on your behalf. We must receive your submission within one year from the date services were received for payment to be considered. See our Out-of-Network Coverage Tip Sheet for guidance on out-of-network coverage.
